Our In-home Nursing Services

Skilled Nursing Care

Let LRVNA’s highly skilled nursing staff help you recover at home. We provide medication management, specialized wound care including wound vac, IV therapy, physical assessment, chronic disease education and management, and more.  Please call 279-6611 for more information about any of our home health care options.

 

Physical, Occupational & Speech Therapies
A skilled physical, occupational, or speech therapist can come to your home to help you recover from a surgery, illness or accident.  Our physical therapists are skilled in current technologies and therapy for surgeries such as makoplasty, arthroscopy, and total joint replacements.  Our occupational therapists can provide therapy for breathing, daily living skills, shoulder surgery, and to regain skills.  They provide support for older adults experiencing physical and cognitive changes.  Our speech therapist will provide therapy for swallowing disorders, expressive communication, pronunciation; memory loss related to word finding, confusion, stroke, traumatic brain injury, accidents, head injury; MS, or ALS with communication challenges.

 

Medical Social Worker
Our MSW’s are dynamic, caring individuals who can assist you in social and emotional factors related to your illness.   They can assess and provide information about financial resources and community resources available for you.  They can also provide services on a short-term basis to a family member or caregiver to aide in the effective treatment of your medical condition.

Multi-Disiplinary Case Management

Our whole team approaches your care in a holistic way, and develops your plan of care with all disciplines and your needs and special circumstances in mind. Our team consults with you and your doctor and involves you in your plan of care.

 

Health Education
We offer clinics and one-on-one help with Advanced Directives, nutrition, community resource information and other needs. Please call for information and with questions.

 

Wound Management & Assessment

Pre-hab Therapy

Medication Teaching & Management
It is important to understand the different medicines you are taking.  Our nursing staff will help with medication teaching and management.

Infusion Therapy / Injection
Infusion therapy involves the administration of medication through a needle or catheter.  We have licensed nurses on staff that can help with infusion injections in the privacy of your home.

VAC & Other Specialized Wound Care

Licensed Nursing Assistants

 

Our Licensed Nursing Assistants meet a wide range of patients’ home health care needs including:

Bathing, dressing, personal care, simple wound care, management of medical apparatus, emotional support, medication reminders, home exercise program help, homemaking support, and caregiver respite.  Our LNAs are licensed by the state and supervised by a Registered Nurse.

Blood Pressure, Foot Care & Flu Clinics

 

 

 

Blood pressure and foot care clinics are offered on the first Wednesday of every month at 12:00 at the Meredith Community Center (Senior Center).  BP is free and foot care is a $10.00 donation for those able to pay.  Please call 279-6611 to sign up. If you can’t make the clinics, call our office at 279-6611 and we may be able to help you at home.

Newborn Well Baby / Pediatric Nursing

 

Most new parents have lots of questions.  The LRVNA has nurses that can come to your home and help with many of the issues that new parents face included pediatric nursing specialists.

Treatment Options in our Office:

The LRVNA offers professional home health care services, making it possible to receive comprehensive health care services in the comfort of your home. Working with our clients and their physicians, we develop a customized home care plan to reach recovery goals, optimum health and independence.

For those who are not homebound requiring some skilled nursing care or therapy, we offer services in our Meredith office:

  • Protime/NR (with a Doctor’s order)
  • Blood Pressure monitoring

A nurse is available at our office Monday through Friday from 9am-10am & 1pm-2pm
